How can I protect my son from Commotio Cordis?
My son plays hardball and I was wondering how I can protect his chest. Commotio cordis is the condition of sudden cardiac death or near sudden cardiac death after blunt, low-impact chest wall trauma in the absence of structural cardiac abnormality. Ventricular fibrillation is the most commonly reported induced arrhythmia in commotio cordis. Blunt impact injury to the chest with a baseball is the most common mechanism. Survival rates for commotio cordis are low, even with prompt CPR and defibrillation.
Ryan - I can understand how you, as a parent, would be concerned about commotio cordis. Such cases are often reported on the evening news and are invariably tragic. Although it is not a common occurrence, it does happen. Unfortunately, short of pulling your son from sports altogether, there are not a lot of proven ways to reduce the risk of commotio cordis.
What we do know is that the risk of commotio cordis is increased when the object striking the chest is harder. The softer the ball, the lower the risk of a blow to the chest causing sudden death. Unfortunately, at competitive levels, this is not something that can feasibly be changed.
Although chest wall protectors may prevent traumatic injury in some sports, the evidence to promote their widespread use for the prevension of commotio cordis is lacking. In one recent study, 38% of competitive athletes who experienced commotio cordis were actually wearing commercial chest protectors at the time of the event, although in 78% of these cases, the chest protector actually failed to adequately cover the chest area at the moment of impact.
It is important that you make sure your son knows how to keep his chest protected from blunt impact. Admittedly, when kids are caught up in the competitiveness of a game, this instinct may slip their minds. Therefore, most importantly, all sports venues should have immediate access to an automated defibrillator and someone trained to use it. The sooner a person who suffers commotio cordis is resuscitated, the higher the chance of recovery. Most times, people wait too long to apply the defibrillator because they think that the victim simply "had the wind knocked out of him", so athletic trainers and coaches need to remain alert and assess anyone who collapses on the field promptly.
